When writing letters to people to request evidence it can be tricky to get started or find the right structure. To make things easier for you, here are some sample letters you can use as temples for your appeal.

These are two letters you could use to help you ask professionals for evidence of your mental health condition. The first relates to Employment and Support Allowance and Universal Credit. The second related to Personal Independence Payment.
